This is a simple guide that I created as part of a way to help give some advice to heart patient's leaving the hospital and going home. I never felt like I was completely informed about what to expect and I started thinking that some form of hand out would benefit a caregiver and patient experience. There isn't any earth shattering medical insight here, but it just might benefit someone looking for some hints and strategy. Maybe even a little inspiration to help the process along. Been there - done that.
